The two of you would either share your Great-Great Grandparent (Born between WWI and WWII, if you're an 18-29 year old) or your Great-Great-Great-Great Grandparent (Born between and 1875 and 1900).
And given the relationship, unless one of you had either one or a couple of later-aged paternal ancestors you're looking at a 40-50 year age spread between the two of you.

Even with Count Morado schema I fail to see who this can be.
I've 2nd and 3rd cousins that I know, but I'm now 51yo and haven't seen them since near to 30 years. So the grand children of my 3rd cousins, I'll not even know they exist.
Plus, there's apparently two generations difference, what mean that the age difference would be the same that with MC's grand daughter... Not interesting as incest relation, nor even as none incest relation.

Apparently it's a bit open to interpretation depending on where you live, since the term just means 'relations between people who are too closely related' where 'too closely' is defined by law.
But your example seems like a very far-off family relation, so I don't think it'd legally be considered incest. The more you know!

This is for the USA; they even extend the law to step family and other stuff we see devs using to get around Patreon's T&C.

I'm not Patreon's T&S team, but here's my guess: Patreon might consider it incest. Fans of incest porn almost certainly would not consider it incest (it's about as much incest as marrying somebody of your same race from your hometown would be, which nobody bats an eyelash at). So you lose both ways unless I absolutely don't understand what you're trying to do.
Second cousin would be too distant for me to really consider it incest, unless the family was weirdly close (like basically all of Austin, Nevada) so third cousin twice removed doesn't even begin to register.
1. Legally no most countries' laws would have no issue with it.
2. Socially most people are fine with 2nd cousin and beyond. In truth the vast majority of people would have no idea who is their 3rd cousin twice removed or anything beyond 2nd cousin.
3. As far as what some website might think, no idea. Half the time I think they go out and survey crack addicts for their ideas.
Then I realize not ever crack addicts are that fucked up.
The further out you get, the less anyone cares. Third cousins are often about the same age, but that just means that you share a set of great, great grandparents. In most families, that's little more than a stranger.

Most traditional incest stories and laws generally state that they have grandparents in common at the maximum. That's siblings, parents, aunts and uncles, first cousins and grandparents. That's generally when it's considered incest even if marriages between first cousins are legal.

Personally? No. I don't even know any of my family half that far removed, nor have I ever cared to.
The only way that would possibly feel like incest would be if you grew up with this person as if they were no different to any other cousin.
