It's anti-wish-fulfillment, if the guy doing the cucking is normal or good-looking, he's easier to identify with, which could make it feel more like netori regardless of who the PoV character is. As for why White and Asian guys get turned on by watching big black guys cuck girls, it's a long story.

Also, people who like NTR tend to have a fetish for messed up and taboo stuff, and what could be more messed up and taboo than an ugly old man imprenganting a young beautiful girl while cucking some poor sap at the same time? Just don't go conflating fantasy and silly porn with reality here, it's scary how many people can't seem to tell the difference.
