Yeah many scenes are not triggering with the current version on android at least. Even ones I already unlocked

Are the death forms actually random? I can't get any of them to appear again, and I'm missing a lewd scene with the three-headed looking one. The Words list for Cup of Death doesn't work either, it just does nothing or it softlocks the game when you try "Transform" from there.

No. Or, it might be bugged, for that I'd advice you to just restart the game (not reset your game data);
The death forms (Grim reaper Horu form, if that's what you're seeking for) appear after a certain amount of deaths. I don't remember the exact number, but the information about how to unlock them is in a Google Drive posted in this same thread. Also, there's no "transform" cup in the game. Transformations occur after certain combos (like Immortality + Sun).
Seems like not, mate. Dev hasn't said much on patreon, and it's likely that it won't be any near future.
If it is a future I guess I'll swallow my own words, buckle up and roll into a ball to cry.

Can't seem to figure out how to install... downloaded on to android and extracted the files... can't see a file to run it... can anyone help as I'm an idiot.

Also, I didn't say anything about a Transform Cup, I meant the "Words" menu in the upper left. It lets you pick a word, view transformations tied to that word, and Transform into them manually. But the Death cup doesn't let you (except the zombie girl form), because it seems bugged. Either nothing happens, or the game softlocks and needs to be closed.
Ah, yeah, that's likely because the dev fucked up the code, since the Death girl transformation isn't tied to any particular cup (like the rest do) but is rather a "status effect" that's only dependent on the amount of deaths. Hence, you can't pick it since it doesn't figure in the code.
