Npcs have a schedule they follow that changes as their life changes, News that changes like maybe the news says an exhibitionist has been spotted recently you could find said exhibitionist or it turns out the news was talking about you. Stuff like that

that add reputation and extra dialogue, including for lewd MC. OTOH it's been updated by Bethesda again recently, which adds to the clusterfuck of finding the right versions of mods.

If you don't mind text-based games then Degrees Of Lewdity has quite a reactive world depending on what kind of things you do. Your reputation influences other NPC's as well as some flavor text eluding towards your shenanigans.
