That scene seems to be bugged in the 1.08 mtl, the lizard will not use any sexual attacks, only spamming fire & normal attacks.

You can only get to it through the teleport item. I think it get's unlocked at the end of the game.

Fight with the lizard let them lick her p*ssy when the kid say enough then come out the dungeon

They only do a mix of attacking and fireballs.
