Seeking Body Modification - Heavy

I am looking for more serious body modification games. Things like magic alteration, extra "parts", size alteration, parasites, and more.

More specifically, I'm looking for games where I have some degree of control over these aspects. Of course, I don't mind if some of these points aren't hit. I know that I am looking into a niche genre, but I'm hoping that there are some that I haven't discovered. Feel free to list anything that comes to mind.

I don't do Viz Novs. I'm very much into being able to take things my direction.
