Identify can somebody help finding me this game

I played a game where we play a couple, in the begining i remember the gf lets a doctor touch her in a examining for her sprained wrist(you choose that to let him or not), and going to mc's hometown where there is his big sis that obsessed with her brother and later in the game that i remember mc goes in prison and his gf fucks some of the bullys in prison for her bf etc.
i think game made with renpy

Sounds like Where it all began. The game has been remade, but the parts you describe happen in the " Old version v0.6 Before rework" Just check the spoiler for the old version download.

The new version doesn't have anything good in it yet.
