There's a few one-time unlocks (nipple ring piercing, nipple nametag piercing) but even most of the unlocks are just more exponential multipliers.
Nothing really changes beyond numbers getting bigger; there's very few visual changes for additional levels of any drugs other than the "boobs get bigger" one.
Towards the end there's some dialog around her uterus and ovaries being yours, but it's just teasing.
I still played it and enjoyed it because the fetish is almost exactly what I want ... but it would really have benefited if the developer had asked "ok, but then what?" a few more times.
Also ... the womb tattoo should really require or cause removing the relevant articles of clothing.
