Most games with that type of setting are also heavy on fantasy like magic, dragons, potions etc. If that is your thing then just do a search for Fantasy and you'll have plenty of options to choose from just ignore the sci fi games cause they get lumped togethr lol. As far as realistic ones then probably Esteria and To Be A King. Realistic is used loosely but definitely way less fictional things. A Knight's Tale is also a good choice but if you are in it for the story then this one isn't good. Decent characters and some decent lewd scenes but the story aspect kinda got pushed to the side lol.
