Longest game with lewd scenes I've ever played: Kamidori Alchemy Meister
Japanese strategy RPG/visual novel with a focus on grinding/crafting. A quick playthrough of it is probably like 50 hours. Hundreds of hours to do a completionist run. I remember it having solid RPG mechanics but a fairly forgettable story. There are only a few lewd scenes as far as I can recall.
Honestly, I lost interest probably 50-60 hours in.
There are also some very good and extremely long Japenese visual novels. If you have any interest I would recommend Type-Moon as an entry point:
Fate/Stay Night
Tsukihime
The stories are definitely angled towards the young adult market.
Both of these require multiple playthroughs (I think 3 playthroughs?) in order to get the entire story. There are dozens of hours of content per playthrough. The story changes each time you play through it. I remember all of the playthroughs being good. Each playthrough will have a different lewd scene (read: Just one scene per playthrough) with the female deuteragonist/romantic interest for that playthrough -- a different one each time. Side characters become protagonists and visa versa.
Fate/Stay Night in particular became wildly popular, was re-released for Playstation without the lewd content, and went on to spawn a variety of spin-off games and anime series with no lewd content. I believe it is the highest rated visual novel of all time. Most of its fandom are only dimly aware that it started off as a hentai game.
Most western lewd games are indie productions and tend to be on the short side.

Few of my favourites are long. I also like LONG games and looking for more.
(not including VN, where I consider them not games you play, but games you mostly read and few choice options)
Wicked Rouge, is very long if you want to grind for better stats. like days and weeks long. The new version is shorter at the moment (also the training has become shorter)
Cost of Survival, can be long, still long even for story mode.(since there is no skip days)
Haramase Simulator, also grind yet fun, very long.
Brother King, also very very long, grind.
Strive for power, grind very long.
Treasure of Nadia, also long hours.
so many more where I spent days and weeks, where I am actively involved and mostly fun grind.
