So I have this terrible habit of thinking I will remember a title and I can't.

This one is male protagonist, 3d realistic style, multiple endings I think renpy but maybe unity. the only ending I had gotten was 2 guys go after your sister and you kill at least one of them but take a baseball bat to the head and have memory problems even worse than I do apparently. I think you were a boxer or something? maybe just got in fights alot.

Not sure if this will help but I still can't come up with this game and its bugging me. I think it must be missing some tags in the original thread because I have searched and searched.

The ending I had got before you are standing in a gym hitting a punching bag and talking to another guy, you tell him that you have alarms set on your phone for everything now because you lose track of time otherwise and you have to go pickup your sister from work. The sister is a waitress and has a GF or wife I keep thinking, guys hit on her and you beat them up after leaving.

That's it! thank you. I was getting the boxer part from the cameo from valentine at the end I think and basically combined the two games in my head.
