I played a game at least a year ago that had a single scene where a wizard (i think) inflicted a cowgirl-ish transformation (I think it was mainly breasts and slight weight gain) on a paladin or knight-type lady who slaughtered a village. It was accessed by viewing a photo in your phone or journal. You could interact with her after at a spa.
