What I mean by consequences are things like them no longer being loyal or having major debuffs that permanently mess the balance. If there are ways to get rid of said debuffs and regaining their trust again, then that's fine by me too.
There was a story event in Sakura Dungeon when one had to "sacrifice" one of the party member to a dragon girl (mini-boss), and then get her back by-mail (or something like that). Heroine Rescue Team has this kind of content more gamified - your companions can get tied up and carried away to bondage devices to be rescued by the player character or other followers.
Not strictly companions, but in the game I'm making the main idea is that a character goes into the dungeon, eventually gets captured and the next character goes for the rescue, rinse and repeat

Leane of Legitimate Crown has something like that. When an ally falls they get captured and eventually recruited to the enemies side if you don't rescue them.
Special Operations Unit Signal Force has enemies kidnapping allies and violating them.
I know there are at least two other games like that, not sure what their called but one you played as two magical girls where your partner gets captured and you have to rescue them.
And another that you play as a trio of women and in two certain areas the enemies will try to kidnap some or all characters then you immediately run a rescue operation.

Kegani Lab's A Village Targeted By Barbarians.
The barbarians would occasionally attack your village and if you are under-defended, take away your chicks (either of the two female protagnonists) and even if you save them within time, they will be slightly corrupted. Keep it up and they will end up betraying you/preferring the barbarians in your place.
Decent mechanics and gameplay but shit storyline (its MTL).
