You need to be in a relationship with good Eve.
1. Eve must not have had affair with Jack. Must have dumped Dave for Ash
2. You must choose to keep Eve good when presented option to make her more naughty.
3. You must choose to be dominant partner in the relationship.
4. You must choose to open Eve up to an open relationship. This can also be Ash fooling around but committed to Eve. Can also persuade Eve to join in 3somes (Men or Women)
5. Alternatively if you have first 3 points you can also choose to cheat on Eve and have multiple or single affair (Ryan for example). You can choose to hide it from Eve.
In all my playthroughs he end up surviving and comes back at the of the game, i didn't know he could die
