It's not that unusual.

The tradition of the child getting the fathers name or even the wife taking her husbands name has stopped being universal for some time now.

Kids born of parents that aren't married have been known to take the mothers surname. I've seen and hard of more than a few women keeping their own surname when married too, especially celebs.

My wife kept her name - that was her choice - our kids have her name as an additional middle name to keep it alive also.
It's fairly common to make it up as you go along here, no laws against it in Canada

It's pretty much standard practice for developers utilising Daz3D (which is the modelling program used for Grandma's House, among many other games) to use whatever basic models are available for free and then tweak them a little to better suit their needs. That is probably why you recognised the nurse's face, for instance.

No need to pay homage, the assets are free Daz assets, some are paid for assets, either way the creator of those assets gives access for them to be used. Very, very few devs use original characters. Most are tweaked a bit by the dev to give them some difference but the base models are all available for use by anyone.

Yeah, that is about as accurate and succinct as a description of this game can get. The only thing missing is that it also has a really dumb and silly sense of humour, which is all good in my book!
