I recall playing an RPGM game where the (shota) protag was isekai'd in some form or another. After going on a short intro quest through this mountain area, you end up in a town populated by female elves. One of those 'only male in the world'-type things.
There was a farm area with several NPCs working you could talk to, and a decently well-populated world from what I saw. IIRC there was like a bar(?) you could go in, and talk to an elf to immediately get a blowjob from her if you wanted. That only stood out because it was an immediately accessible one. I believe you were also staying in a house with an elf, and so there was going to be a whole caretaker/incest-ish thing happening with her.
