"Helping the Hotties" doesn't know what it wants to be. It's not a harem game, but also doesn't offer exclusive paths. To make matters worse, it's impossible to be faithful to your existing girlfriend, or to act in any professional way. Outright rejecting someone isn't possible.
The MC has a huge weakness, as soon as he gets an erection, he becomes uninhibited and looses the capacity to think clearly. There are 6–7 women who gladly abuse this fact, and sometimes force the MC into a sexual encounter. Once, quite literally, where the MC gets tied up and blindfolded, then sexually assaulted by one of the women. But, don't worry, after finding out who did it, he claims to have liked the experience.
The flaws in this game's structure become apparent when you try to play as someone with even the smallest moral compass. You will end up giving in to women who are under your care, who you're responsible for. And you will have to deal with the MC second guessing this decision every time, yet learning nothing. As soon as his erection happens, he's DTF. To make matters worse, the MC is actually majoring in psychology. I guess he slept through every ethics class.
At one point, the MC says "I just don't want my dirty laundry aired to patients that are supposed to view me as an authority, you know?"... While he's potentially been naked with all of them. He says this kind of thing often, something ethically right, while his actions contradict it every time. This way, he comes across as double-faced.
During the final scenes of the game, the narrator states "Fortunately, your integrity has remained intact throughout the whole week, your actions admirable to say the least.". I wonder if this is a jab at criticism, I refuse to believe that the devs are entirely tone-deaf.
The redeeming factor of HtH, is that it's a gorgeous game. The scenes and animations are of a high quality, as are the models for the Love Interests. The four patients each have a unique personality, and show characters growth as the game goes on. Emily, Kiara and Eve, however, are a different matter. Emily and Kiara are your colleagues, while Eve is the therapist of the camp. Eve doesn't know what she wants either. She'll be suspicious of the MC's activities with the girls, but also allows him to become an assistant-therapist and have one-on-one time with them for therapy. She'll also end up seducing the MC, who will just give in, because boobs. Emily and Kiara are both interested in the MC, but most of the game this doesn't really progress. In fact, most of the game we have no idea where they are or what they're doing. They're just there when some B-plot needs them.
