Installing an "incest patch" on a mac?

When downloading the "patch" is comes as an .rpa file, normally on a Windows machine you have the option to dig into the game folder and add it in here, however, with a mac this doesn't appear to be an option as the file seems to be self-contained as one file.

Does anyone have experience with this and is able to help?

Right click -> show package -> find "game" folder.
