I've been having a bug in v9Bj no mod, but I saw it in older versions too:
Sometimes during combat, you can only target one guy with attack, sex skill... Even if more are present. From memory, it's always the 2nd guy from the bottom (I could be wrong with that).
It doesn't happen often enough to be game breaking though. It's just random on some trash fights.

Anyone else has seen that?

Sounds like a case of "I forgot to move my mouse pointer away from that spot" - the game (or maybe the engine as a whole)...does something I am not tech savvy enough to explain. It basically prioritizes the mouse pointer over, say, keyboard buttons, and when you hover the pointer over a selectable area (e.g., skill buttons or enemy portraits) keyboard navigation becomes useless until you move the pointer to a spot with nothing to click.

Maybe if I find the time, I will look to make a more in depth guide about Pure Prisoner Mode run.

Now for some basic stuff that I like to do (reminder, that is only meant for Pure Run): Never ever fight against guards. Get the +10 Agi and +10 Dex Acc early (you can even get them Day 1, makes battles on the 1st Floor easy, since you basically get Stats for 4 Levels). Don't fight in the Bar. Don't increase Order, unless it is about to hit 0, to make more money (as long as your Order doesn't go below 20 you should be fine). Don't fight above 25% Fatique, it's a risk you don't need to take. Don't use Taunt or Flaunt, unless it is really necassary. Get the Problem solving Edicts always ASAP (I tend to go for the expensive ones there but sometimes it depends). On the 2nd Floor get stuff like Skewer, Cleave and Healing Thoughts. Rouges are a High Prio Target, eliminate them as quickly as possible. Same goes for Lizardmen on the 4th Floor.

From all my experiments i learned from this game starting from 0 to day 40, First floor is the most important part of the game you can shape your prison. It is good idea to spend good amount of time there and build up good foundation for Karryn and prison

Well. On Level 1 I get Repair Kitchen once my Order is getting low. So that is +2.
On Level 2 I get, not in specific order: Basic Guard Training +1, Hire Cooks +1, Train Anti-Goblin Squads +2. So that is +6 now overall.
Also depending on how well/bad the run goes stuff like Repair Visitor Center +2, Standard Equipment +1 and Better Guard Meals +1 are an Option.
But I wouldn't take Hire Chef and Bulk Food Supplier. They cost too much Upkeep and give little Control in return.

edit: u cant tactical retreat flee on prisoner difficulty, that's dumb.

Hate to be that guy but technically there are several that fit the bill but they're mostly for doing a good job as a warden. You're definitely not wrong though since those titles are massively overwhelmed by the tidal wave of erotic passives and titles. On top of that, there are no 'clean' passives, only 'clean' titles and that is purely by technicality of not being related to eroticism. Heavy emphasis on 'technicality'. The only title that I can think of that has to do with Karryn's purity is Emperor's Secretary iirc.

As for why some people want to play this game straight as though it isn't a porn title? I'd reckon it's the challenge of it, trying to remain a virgin for the longest time in a setting where it's expected that you lose your virginity very quickly or something like that. Like the Title you get, for getting all Hero Books, that you only can get, if you have low Slut Level? Yes, there are much more Titles, that are Slut focused... duh... it's a Porn Game.
