Its extremely likely, given everything that occurred, that Kaori and her parents were injured / killed in the same accident that took sekai and nozomu, and they used sekais organs for the transplant kaori needed. thats why she seems to be able to just take control of kaori. also why kaori showed up to the roof during last reset dressed as her... dont feel bad, i also initially thought it was little yumi as well lol

Kaori also references 'spare parts inside me' in her hex code speech, which has to be her way of describing donor organs.

This also contextualizes the sounds of life support in some of the early happy scenes, as if Sekai had died on impact her organs would not have been viable, so she was likely in a vegetative state.

And Akira, as next of kin, would have had to be the one to authorize pulling the plug so that the organs could be used.

The accident happened around 8 years earlier, when she was 14, maybe it's an allusion to Sekai possessed Kaori guiding young Kaori, Yumi already said she was "different", probably her mental age was still childish and after the accident Sekai is guiding her.

Yes, she says she has to go to see the doctors periodically to drink the health juice or something, and that she has a spot that can't be touched or she dies when Akira mentions having a soft spot for her.

I'm going to just sort of crap on this theory, but you can't put an adult humans heart in a child or young teen as its to big in general.
That being said this is a fictional work and real world issues might not apply. Also she had crime connections so they might be able to get ahold of a doctor with no ethics willing to do it survival rate for something like that is really low though

Got through the update, and sheesh it left me wanting more. A shame next update will probably be nearing the end of June. Did not expect Kaori to latch onto Akira like that. Nor the eyecolor changing...

I can't wait to see if he tells Maya and Ayana about Makoto telling Ami of the resets. And what comes on it. It seems soon the next phase of this game will be taking the darker path. At least from what I saw earlier about this. How this is the first phase, and then there will be a darker one, and then one for Akira to redeem himself. Something like that. But DAMN is Ami scary, this explains a lot about why always has such weird scenes or seems to act aggressive, like with Haruka that one time. But Akira's phone dying as well, has gotta be some supernatural interference. Even while I assume Ami was the one lying about the phone call

Although the other guy posted it already, I've also done a 0.31 event guide with the triggers for events I found from the game script. I haven't done all events yet but this should be correct. Doing events from top to bottom order from this guide is recommended.
EDIT: I also did this hex translation for events that has it.

How old was she supposed to be at the time of the accident? In the real world 13+ can get adult hearts (no idea what the standard of care in Japan would be though)

The parts that wouldn't happen in the real world: Organs almost never go to the same hospital, the odds that two unrelated individuals in the same accident having relatively compatible hla subtypes is vanishingly small. I also can't say I've heard of heart transplants occurring due to a traumatic injury

She's around 22, and the accident was around 8 years (or around 9, depending on how far along they are in the loop). So, Kaori would have been around 14.

Interesting, and yeah I can see it being Kaori, albeit personally I'm thinking it might be Maya.

They almost kissed recently, Sensei has been trying to seduce her ever since, and if they do anything, it'll presumably break him, which implies it's something he's not supposed to have according to "the world" itself/herself (Maya herself fits that perfectly).

There's also how it was Yasu's god who said that:

HOPE/Nozomu which seems to be Yasu's god has made it clear it wants to devour those that Sensei loves, and it seems likely that Maya is at the top of the list.

Then again, I could be completely wrong.

Sana could also be a possibility, as could Rin, especially since those two might even be related to Sensei if it was his brother who knocked up their bio moms. Maybe it was even Nodoka? It definitely felt more taboo than usual imo.

Also it's probably a stretch, but the shapeshifter herself also might fit. She claims to not be that kind of character, which fits the not supposed to have part, by design. Then add on the possibility that she's some kind of supernatural daughter of his, and that makes it even more taboo.
