I'm looking for a game I used to play where you play a therapist and have clients who you can shape the path of.
There is also an occult side to the game where you craft potions in your basement to influence people, mainly your neighbor.

It included real porn. I can't seem to find it.

No it wasn't that one, although thanks for the reply. It was a sandbox game, I believe HTML. You move in to a new house and find a recipe for something occult in the basement. You have to gather flowers to prepare it, and can also work with patients to choose their path, changing the scenes.
