Ive only played badik, our red string, summertime saga, fetish locator. I usually dislike copy paste games with the same 3D characters but Badik was really good, ill also check out acting lessons. I am basically looking for open world type (NOT NECESSARY), slice of life, choice based games that have a meaning, preferably with unique art style (I am also fine with 3D games as long as its good, I know it might not be badik level). I dislike games where the MC is just able to fuck every man/woman he comes across to, monster games, and games that are just full of huge cock/breast/ass characters. I am also interested in trans and fetish stuff, though they arent necessary but Ild prefer games that include one or two things like that (like the optional trans gf in summertime saga/fetish locator).

The Last Sovereign makes most (all?) choices count - sometimes way down the line you'll get something different in an event because of some apparently irrelevant choice you made 2 weeks ago.
It also covers your point about ridiculous MCs and NPCs - it features one of the best crafted MCs in lewd gaming history, and drawings are realistic (not in terms of photorealism, but in terms of bodies).
It doesn't feature/focus on futa nor has an open world, but it is very much worth playing.

EvaKiss' first game 'Good Girl Gone Bad' is pretty similar to Red String in the fact that choices do matter. Granted, the characters are very stereotypical and not very deep, but on the plus side there is a trans character.
'Nothing is Forever' is another game in which choices have real weight to them. It's one of the few games that makes the player feel conflicted about relationships. The renders aren't the best but if you like good writing and meaningful choices, it's a must-play.

If you want a game with lots of meaningful choices, I really suggest College Daze. It's a real porn game, I know many people dislike them for some reason. There is exactly 0 grind.
You can really decide how your relationship with all the characters evolve. Most of the events are written differently depending on your previous choices.
You can also decide if you just want to focus on the characters or if you also want to explore the lore of the story. It is not an open world, but it is more open than most sandboxes.
You will be able to replay the game many many times, trying to play it differently, and you will really likely discover something new all the time (depending on how much you limit your choices due to your preferences).
You can play it with or without incest. There is a an energy system (deciding how many actions you can take per day) and a simple stat and magic progression system. There are integrated cheats if you want, one that just remove the energy system and keep the progression system (this is the mode I use, allowing me to manage my stats and magic without the energy limitation) the other one maxing all your stats, magic money and energy.

Give a try on mine, our red string, good girl gone bad and solvalley school was my inspiration, i don't think i can even scratch Eva kiss level but i tried.
