One of my favorite games; while the plot obviously requires a significant suspension of disbelief, the graphics are very captivating and the game, overall, is interesting.
Graphically, Eva is extremely well-executed; one of my favorite protagonists.

The rating based on 0.8 update.

First of all, the game description in the OP is misleading. This game is not a NTR optional game but a NTR focus game with option love path(which is nearly disappeared).

The game has stated that there are 3 paths in the game: NTR, NTS, LOVE.
In fact, the point system for path variation in the game is abandoned by the developer in this update.
1. The NTR content reduced from husband NTR(with co-workers/patient/daughter) and wife NTR to just wife NTR content which she is just fucking everyone.
2. The NTS content reduced from husband NTS(wife share husband with other woman, possible cuckquean/FFM content) and wife NTS(husband share wife to other man willingly/ love watching wife perform sexual act with other man) to wife NTS content which husband just being a cuckold to watch wife fucking everyone without any action(nearly none) with wife.
3. The love path content just completely disappeared in the middle and make a comeback in this 0.8 update.
4. I nearly forgot to tell you there is also the daughter path with all the NTR/NTS/LOVE content which the NTR/NTS content obviously remain the focus in daughter path, She did have a scene with her brother but the incest content is not the focus. The father/daughter scene possibility seems to be thrown away.

The whole story from the start is clearly set up for wife NTR/NTS content. By progressing the story, you are required to play from nearly all family member perspective like husband(mc)/wife/daughter/son, even the sister-in-law. The way the storytelling nearly lost all the focus by switching player perspective so frequently.

One of the main focus of this 0.8 update is the comeback of the love path which is secretly abandoned by the developer in previous update without any public announcement. The same situation as the developer switch the game focus solely on wife NTR/NTS content. The love path content comeback in this update is a disaster cause it has no difference compared to his previous or even worse. All three paths are set up in the same event and story again which the event and story is completely set up and favour for NTR/NTS content which the love content is clearly not fitting. What a joke.

This 0.8 update clearly made this game completely a joke by abandoning all its previous point/path system which make a potential good game into another classic ntr game with meaningless love content. The reason I didn't give this game a one-star review only because of the wife model which I like a lot.

I heard this game may develop a season 2. If the developer keeps using the same kind of storytelling like in this update, he had better just fully going towards the wife NTR/NTS, instead of giving us some shitty love path content or other NTR/NTS variation which he clearly didn't really give a fuck.

Honestly this was a bit boring. The plot was just a day in the life of this family, with a lot of porno scenarios that usually made little to no sense.

The choices basically boiled down to: you x wife, wife cheating, or nothing happening.
The daughter was not an option, you just got to see her side story fucking around.

It had potential but really squandered it left and right. No story, no relationship growth, not goal. Just day in the life, porno style.

This is an NTR game. By that, I don't mean that this is a game with NTR elements or optional NTR as it advertises. It is centered almost entirely around potential NTR scenarios. Dialogue is also clunky and a bit unnatural, but overall, my feedback is to include elements of love and lust more throughout into the game rather than scene after scene after scene of MC's wife being potentially seduced or his housemate being taken advantage of by older men for being unaware. Even if you avoid all the options labeled NTR the whole environment is just... unnerving.


My constructive feedback would be to include more scenarios of character development and relationship development besides other guys trying to bang MCs love interests. If really into NTR and don't need interesting characters, this might be fine for you. If not, run.

I usually avoid NTR altogether, but the Wincest tag in this game is outright misleading and feels like a complete sham.

I don’t completely rule out NTR games if the player can avoid it through meaningful choices, or if the third party involved is a female character. On paper, Medical Special Care seems to offer that freedom, with three advertised paths: Netorare, Netorase, and Love. Unfortunately, that promise falls apart very quickly.

In reality, the game is overwhelmingly committed to NTR, with around 90% of the content focused on Netorare/Netorase. The so-called Love path is extremely barebones, offering little more than generic vanilla scenes between the MC and his wife, with no real narrative depth or payoff.

The Wincest tag is especially deceptive. The game explicitly asks the player to define Karin’s ( Dau****r )relationship to the MC, creating the expectation of meaningful interactions. However, the game then proceeds to deliver almost none. Out of roughly eight episodes, I can count the MC/Karin interactions on one hand—and still have fingers left over. Even then, those scenes are limited to brief greetings at home or short hospital encounters. Karin feels like a completely separate character, following her own disconnected storyline with almost no emotional or narrative integration.

The game presents choices that supposedly allow the MC to protect his family from NTR via the Love path, but that’s where the illusion ends. If the Love path were meaningful, it should logically build a love or relationship stat between characters. Instead, the stats screen exposes the truth: there are only Netorare and Netorase stats. There is no Love stat at all. This makes it painfully clear that the Love path exists only as a token option, with no real rewards beyond the personal satisfaction of gatekeeping your family’s private parts from horny doctors and patients..

Structurally, the game is boringly linear, with each character locked into their own straightforward route. It feels like three separate stories running side by side with minimal interaction:

1. MC and wife — Netorare / Netorase
2. Karin — Netorare / Netorase
3. The home storyline, where an aunt and two additional siblings suddenly appear with little buildup

This game is undeniably a haven for hardcore NTR fans. However, if the developer has no intention of expanding on inter-family dynamics or providing meaningful interactions between the MC, Eva, Karin, Dean, and Lana, then the Wincest tag should be removed. As it stands, it only wastes the time of players who download the game expecting content that simply isn’t there.

Usual set of plastic dolls pretending to be characters go thru the motions.
Lines of boring non-exisitng plot as an excuse for the next set of mediocre renders.
If you are looking for a simple fap game and dont care about the quality much, or really enjoy ntr, this one might be for you.

Nts / Ntr in this game is top notch bcuz of female model... Need more scenes And addition of some renders in old ones like in the bus... I was expecting her to get fucked but she didn't... So yeah just insert it... On unexpected situations to make it hot & spicy

To be honest this is one of a kind game since its primarily focused to medical field and I haven't seen other games the same like this, only like a partial part of it which is the nurse or doctor doing exam to the MC and that's it.

I know the Dev is really doing their best to create more content and I am not complaining if there are flaws to this game, it may have but not too big of a deal. I can't just complain and blame the Dev should do like this and that since the Dev is not like a big company or earning much revenue for us to demand so much. I don't know if the Dev do this alone or with partners but hell it looks so damn great.

What important is, there is some progress and constant updates. I hope this won't be abandoned in the near future since this type of niche and focus haven't yet been from any game. I actually try to find similar games near to medical and I haven't seen any near to it.

They are doing a great game and the female characters especially Eva so damn hot ngl and from that I know the developers really created the character so well and I wont forget Karin as well you got my thumbs up for that . I'm dropping some ideas and suggestions at the discussion thread since I'm only reviewing the game here.

One of the best games I've ever played. I hope the content is still less than 50% done since I wanted to see more. The quality of the game is perfect. I'm looking forward to this in the future. Great job developers!

Gd game imo. Not a ntr fan but u can go with non ntr route to enjoy, though the ntr route has more content cuz this is supposed to be a ntr game by default ig. So, all in all gd art and scenes like them encouraging author to add more stuff and keep on going with this project.

Among the best I've played. The girls are beautiful (specially Eva), the paths have substantial differences among them in terms of content. The plot is nice and well structured. There is a great amount of anal. I mean, for me, there is nothing to complain.

Biggest problem i had and was almost enough to stop playing is i play on a Mac....high quality with 45" monitor. MOST scenes and characters are very very dark. Cannot make out most details. Not sure if it is the same on PC's. Increased all setting on puter to see as a normal picture or video....but sex scenes so dark cannot see anything mostly. Please improve the animation. Please update and finish the game. I know the cheat mod is not yours....but some directions to get it to work on a mac would help. A walkthrough for each of the main choices would help. Overall good...but not on a mac.....

very rarely have i conjured up the term 'low brow' during my half century of years on this planet, especially when i can think of many other ways to describe something that invokes images of a steaming pile.

the imagery is really not my thing. hourglass figures that look a little too pinched and none of the females look particularly 'fantasy' enough to hook me in.
this is coupled with a script fresh out of a porn channel. certainly i found no signs that the writer has even read a single erotic novel....a comic is probably the closest thing. i can forgive shit translations as there are always signs that the depth is lost in translation. reading this was just a mental flossing.

Cheap and basic sounds with no music, choppy and terrible looking animations, and god forbid the terrible porn plot. Even if you are into NTR the fact is this is terrible quality and you can find better out there.

Concept is great it just isn't very well executed. The way the game is animated is a huge turn off. Using multiple still renders for the lewd scenes would have been a better option. Some of the dialogue seems a bit unnecessary but I see what the dev is trying to accomplish. I like that there are three separate paths that are well identified. I know there are quite a few people into the bimbo models but I'm not one of them. I may come back to this one in the future but for now it's a hard pass for me unless the quality improves.

Very hot game with an enjoyable sharing path, Eva is a minx! The game does a good job spreading kinky scenarios out between the larger mystery plot. Stat tracking that seems to offer a lot of player agency, though I haven't gone through each route or checked a walkthrough yet. Looking forward to seeing how this one develops.

It's a game with a seemingly simple story, but when you start it you realize that it's not quite like that. The story has some mysteries, especially around the hospital, since until now it's not possible to know what they really intend with this program. The characters are all very beautiful, especially the wife, and the scenes are good. The game has choices and routes to please different players. Therefore, it's a good game that's worth playing.

This one is not terrible, but definitely below average. First of all, the story is full of errors on the language, which is understandable, but it's also full of nonsense all around, which is not so. Characters make no sense most of the time, with abrupt changes and reactions that are not believable at all. Choices matter as there are three paths to follow, and they do guide the story in different ways, which I think is great.

Render quality and character models are in the mediocre range, and animations are directly poor. Sound is absent and I got a few bugs with missing images and things like that.

So, all in all, not really worth it.
