Here's my list of some dark gore hardcore H games with ACTUAL great gameplay :
Lona RPG --- First of all this game is hard. Like, crazy hard to get into. Lots of mechanics to get familiarized with, walls of text as tutorials, etc. That being said, if you're brave enough and really want to dwell into this high quality game with really extensive life time, this game's right for you. Has corruption, graphic violence, prostitution mechanics, great combat mechanics, great rpg features and is pretty much a sandbox. Your experience with the game will vary according to your choices.
Sinisistar 1 & Sinisistar 2 --- These two are a perfect choice for the sexual gore enjoyer. Where Sinisistar 1 was the concept, the sequel brings it to the next level by going deeper into the first game's mechanics. I have never seen better art than in those two games, truly. Ambience is really well made and game can be a bit hard sometimes but it's still acceptable. In terms of gameplay, it's basically a 2D side scroller RPG. The sequel has some quests and items mechanics where the first game had a really obvious "level" system one after the other. It kinda goes like this : you enter the level, you fall in love with the beautiful art of everything surrounding you, you make two steps, encounter a new enemy, you could kill it but "meh" you want to discover a new h death scene, see what the dev cooked for you, so you die, get the scene, try again, repeat this with every enemy because damn it keeps getting better, finally reach the boss at the end of the level, you guessed it, you get the scenes, after some struggle, you beat it, next level, repeat. The best gore game there is out there no hesitation.
Fear & Hunger & Fear & Hunger 2 : Termina --- While not being centered about sexual activity at all, F&H is the best dark fantasy game. It's as difficult, not to say more, as a Dark Souls game. Fear and Hunger and its sequel, Fear and Hunger 2: Termina, are brutal survival horror RPGs set in dark, nightmarish worlds. You will navigate terrifying environments filled with grotesque enemies, managing hunger, sanity, and resources. The turn-based combat is punishing, with a focus on strategic choices and body horror. Both games feature deeply unsettling atmospheres, complex puzzles, and existential themes. The sequels’ darker tone and expanded lore offer an even more intense experience. For fans of challenging, atmospheric horror, these games deliver an unforgettable, twisted adventure into madness and despair (truly, it kind of changed me, it's just really, really brutal. trust me). Night Of Revenge --- What i really liked about Night Of Revenge is truly just the actual player's progression and its playability. This 2D side-scroller souls inspired RPG is filled with different environments, monsters, weapons, and obviously scenes. The scenes are all really great (and gory but hey you're here for that right), art is undeniably really good and believe me when i say the progress is rewarding. Secret quests, lore, boss fights, stat leveling, this game's got something for you.
