It didn't actually
In fact, I had to start with 3 penises with the same size prompt, then edit the one in front to be small
I also tried to edit the one of the right to be larger, but AI is random and I thought this was good enough

For some reason, all concepts of penis size difference or small penis humiliation are based around having exactly 2 penises on screen, so I had to be creative and iterative to get the above

Not enough reference material for training, I imagine. I am fairly certain that if I went to any real porn or rule34 picture site and searched for that kink, it would be predominantly pictures of only 2 men.
I am more surprised how bad it still is at doing missionary double penetration (even with specific LoRAs). That guy's torso disappearing into the bed — not the first time I am seeing that.

Another interesting example is if you asked ChatGPT to generate a glass full of wine (to the brim), it would always give you a glass half-full. No such problem with, say, a glass of beer.
Edit: Which makes me wonder: would ChatGPT be able to generate a watch with it's hands not showing ~10:07?
