Personally I'm fine with any or all of those except the "becoming the town bicycle" part. Most other exhibitionism games already have that. I hope this developer doesn't go down that path too.
I had the same thought seeing the title then reading that bit. You're not embarrased about being naked if you're an exhibitionist. They're mutually exclusive mind states.
They are not though, you can be turned on BY the embarrassment.
If you're embarrased by it, then you're not an exhibitionist. People that are exhibitionists aren't embarrased, they're turned on by exposing parts/all of their body. You can convert from embarassed nude to exhibitionist, but you're not both at the same time.
This is just wrong. You can be embarrassed while also being turned on. There is nothing about embarrassment that prevents arousal, in fact embarrassment can make it even more arousing.
I have to disagree with you completely, there are both bold and shy exhibitionists. Both of which have their appeal. To be an exhibitionist, you just have to be turned on by being seen, you don't have to be bold about it. The embarrassment can be part of the turn on. You could be new to it, or afraid or getting arrested, or any number of other reasons.
In fact, sometimes it is the shiest ones, who have the strongest tendencies.

The only criteria for someone to be an exhibitionist is to have a desire of exposing themselves in public for the thrill of it. That's it. It doesn't matter if the exhibitionist is the type to shyly hide away out of embarrassment or the type to boldly show it all off, it's still exhibitionism either way because they're naked in public.

Can't wait for the dev to include NPC reactions. Hopefully they pull out their phones or something. Maybe call the cops as some sort of game over mechanic?
Well, if they call the cops, you should have time to get away. Maybe it could start a timer, and you have to go hide.

More accurately (if we are talking about the psychological definition) an exhibitionist is one with the compulsion to show their genitals in public. It doesn't matter, as you say, whether they are shy or bold about it.
The non-psychological (i.e. "common English) definition is anyone who likes to draw attention to themselves by their behavior. In this definition, they are choosing to act this way. It's not a compulsion.
This game is using a well-trodden trope. I hope the dev can do something different and interesting in this trope. Another typical exhibitionist game will have a hard time selling itself with so much good competition.
