It is an old game, around years ago, back when Overwhored, Noxian Noghts, Ouroboros, LoQO I/II, The Last Sovereign and other "more-than-just-porn" games were in development.
I don't remember a lot about the game. The MC wakes up in the wild and wears a brown cloak. You don't see their face. I think you help a lady out at her farm before arriving at your fircy city. I remember one of the early encounters being with a prostitute or something similar. She had dark skin and huge piercings. I want to say it was in a port city and when staying at the hotel, you find letters or something from a previous occupant dictating some of his more perverse sexual activities.
It had a drawn artstyle, not 3D, and it did seem to have a plot from what I remember.
