The first thing that comes into my mind in fantasy setting is anything related to magic, a few examples:

-Female MC or character got an ancient curse that makes them more slutty overtime.

-Succubus are attacking a town by raping all the citicens in said place.

Then we have the monster races aspect:

-Goblins, minotaurs or orcs attacking places to rape all women

-Demons having some sort of conspiracy to develop some evil plan

-If you like the horror genre you can investigate how deep goes the rabbit hole.

And also we have some general concepts related to different races in fantasy worlds such as:

-Dark elves being matriarchal and slutty while normal elves are just prudes

Out of my mind the first two games that I can think of with a fantasy world setting are:

1º Peasant's quest (huge focus on pregnancy and adventures)

Okay, I'd for a change want to see fantasy settings that don't go for budget renfair "European medieval" contexts but are based on a different period or culture.

Sure, it can still be a fantasy (though you said it was real for centuries in your second paragraph). It seems a better fit for netorase, or maybe eventually netorare than netori. It doesn't really rely on the MC's own strength or dominance, just on a weird law. So there isn't much power fantasy in it.

I'd say the amount of fantasising about the jus primae noctis against how little it happens in real life suggests it isn't a very realistic type of sharing, compared to other, less legal types of coercion.

The cool thing about a fantasy world is that you can cram in any fantasy you like if you give it enough reason:

1. Free use: the priestesses of the goddess of love will never deny you sex
2. Incest: a society where incest is acceptable because reasons
