You can avoid pretty much everything.

It's a typical quest based sandbox game.
You either do it or not.

There is a mainquest route i guess, but it's rather boring without the sidequests and i'm actually not sure if you can actually advance through it without doing the given quests now that i think about it.

i am confused some say the update is small, you say the update is big and nearly "game changing".

then i at the images -

it seems there really are many paths covered! but i personally don't like 50% of them (bad luck for me i guess)

i think every scene is and should be avoidable optionally. even with characters of the main story line. cause e.g. i always reject the bill collector and the game (or should i say fooxied) allows it to say "no i don't go this path" which is really cool in my opinion. problem for me is only that i reject the majority of guys i could have scenes with and the guys i would want to see scenes with have either barely or no content but thats my "problem".
