How is the gameplay of this game like? Is it battle rape or only in game over?

Both. Lots of battle struggle and also full CG "lose" scenes. Also, losing doesn't necessarily mean game over, either, she can still escape (but might be pregnant...). Gameplay is like the old YS games, or maybe some of the earlier 2d Zeldas.
