To me it's the character models, especially character models of the love interests.

Milfy City: Especially Mom Character Linda, but others also really good.
DMD: The daughter model.
Midnight Paradise: The mom model.
Dreams of Desire: The mom model.

Most "successful" adult games have these kind of peak character models at least one or two.
What do you think? And lets focus on ingame stuffs rather than update times etc.
