Crusader Kings II, but porn. Alright, I dunno, those games are boring as hell to me but some people love them. I'd imagine that the political intrigue and nation-building is a rewarding aspect in an of itself, and if the heavier mechanics are stripped in favor of lighter, more immediately-gratifying ones, you can incorporate some sexy animated cutscenes, maybe some half-naked warrioress chicks fighting. Okay, maybe more like Mount and Blade:Bannerlord with saucy mods than CKII.

There are multiple porn mods for CK2 already. Out of curiosity I went looking a while back. None of them really interest me but it is a porn niche that appears to be catered to already.

God I've got so many ideas just based on what you've listed Zilcho: a murder mystery gothic horror set in a house that has trapped you in perpetual limbo with a number of house guests, where you must travel into an alternate dimension to access areas, find clues, while avoiding getting drained of your life essence by the many suckling, writhing, caressing creatures of the night and eventually escape.

Assassin's Creed, emphasis on the ass? You follow your ancestor's story shortly after Ezio's death, as you were the only other known descendant with a passing connection to the assassin's who safeguarded an apple of eden. Problem is, your ancestor was a lover, not a fighter, and you have to go through the same bleeding effect, avoiding corrupting the modern assassins while attempting to synchronize with your lecherous, treacherous, and downright deplorable ancestor as he fucks his way across the early 1800s.

As for fetishes though, I'm not sure if it counts, but a slow burn with actually good porno content would be nice. After that it'd be cuckquean, public/hidden sex, orgasm denial (both sexes), vanilla femdom, casual sexual content without having to work for it would be nice further on in a game, freeuse, weird ways to have sex (physically groping a womb/vagina like it's an onahole, nipple/ear/crook of the elbow/knee fucking, sounding). Furries really do be having it all in terms of fetishes, but none in terms of games.
majority of furry games is pretty much as vanilla as it can get, with most mundane gameplay accompanying it, like match 3 and puzzle games with porn. Besides vore i guess, there's lots of that, but "lots" only compared to the the other niche fetishes.

Also rape is absolutely not a underutilized fetish. There is 142 pages of games here with that tag. Many of them are probably just one scene like it is usually be with niche fetish tags, and not a game focused on it, but still, there is a lot. Many actual ones are just probably concentrate in Japanese games and 2D sidescrollers, that is where i see those the most.
