Identify Trying to find game where MC's arms are broken

A few years ago, I played a game where the MC had broken arms and at some point he gets upset and his sister feels bad and gives him a blowjob. I think there was also an animation where her face changed slowly to a sad frown. It's been a while so I don't remember much else.
