trying to find this game

I remember there was this 3d game about a knight or a noble in medieval times, you were a knight and people spoke very formally.
You also had an assistant which you made fun of. it was fairly high quality, probably in unity or unreal engine.
There was also a good and bad system in the game. like pure and evil. the character was male.
there was this funny bit where an old guy dies after sex. and a blowjob scene with a crazy woman.
appreciate the help if someone figures out the game.
