I'm planning on uploading a new game soon and I have a few questions before i do so.
- uploading a new game thread in game request and the game request templates are different. Which one should i follow.
- When it asks for sample, does it want pictures from the game?

The template that automatically loads are for non-developers requesting someone else's game. A developer can also use that template to have an uploader do all the hard work for them.
Yes, samples are images from the game - PNG, JPG, and animated GIFs are common.
