The most interesting part of incest games is... incest, I suppose. 
That said, I like the teasing and seeing a character changing morality or battling with it. The psychological aspect is interesting, if the writing is good. I also appreciate when different paths are available (usually, the more romantic attraction and the more lewd one).

If I had to pick one, since Im a huge fan of anal, I would say where the characters purposely do anal only to avoid pregnancy and the possible problems with the child that would come along with it due to it being the product of incest.

I personally like incest games quite a lot, only for the taboo aspect. Similar to why I love Netori/Netorare games a lot (Read: cheating). I can't do it in real life, it's not me. But the taboo aspect is very attractive (Read: the shock).
But, this is only because I really have a hard time to get invested into characters by default, in many games. So, the fix comes from the shock value of taboo. As Mr./Mrs. Gunizz said, the psychological aspect of it is super enticing (Would you, wouldn't you? What if someone finds out?).

I love incest games. Because of the fetishization of the mother. I love the concept of a woman who cares for you and genuinely loves you sweetly and kindly and then that being cranked into a taboo inappropriate place. mother, son, relationships are the ones that I look forward to the most.
I also like when you get the option of it being a lustful corruption or a heartfelt and meaningful passion.
A secondary taboo and incest I don't mind. Is the bratty older sister getting what she deserves and the taboo of it being a blood relationship adds on top of the revenge aspect.
I love pregnancy content also just in general and the again taboo stigma around it are thrilling to me.

~ Honestly I would say the whole 'Taboo' aspect of incest games stands out to me the most tbh
Like being able to have sex with a women considered your "mother figure" and being able to belittle your dad in the process by stealing away his wife/your mom to be your own personal fuck buddy after corrupting the shit out of her is hot IMHO
And then being able to bang your extended family (e.g., sister, cousins, aunts, etc) adds even more spice to the mix. Overall, I like incest harem games because of the "Taboo" nature of the genre and is quite stimulating from a fantasy perspective for me
