It may inspire developers for future games and give users to opportunity to share recommendations for existing ones. After all, the fact, that you have not seen a porn game with a certain fetish doesn't automatically mean that none exists. As for my fetishes yet to be included in porn games:

- Straight woman turned lesbian. Meaning, as the main focus of the story, the woman in question, who may or may not be the protagonist doesn't really know what's going on and why her pussy suddenly gets wet when a certain other woman is around. Could be done as an obsessions, where she becomes the sex slave of an other woman but could possibly even be done in some sort of romantic way. Ideally the woman to be turned is milf with a lot of straight experience (NOT visualized) but no lesbian one at the beginning of the game. Her son or daughter may make a good antagonist (No NTR incest, they would simply not be happy about their family being torn apart).

- Powerful but secretly submissive woman: A female character who has a lot of personal and maybe also political power, but who has a secret submissive side, nobody knows about. Her submissive trait is strictly limited to the bedroom and she propably kills or exiles everyone who gets a glimpse of it. She may be the protagonist but I'd rather suggest, she is not and the (preferably female) character doing the dominant part is. In a cyberpunk setting this could be a geeky hacker girl, who manages to sneak a micro drone into her bedroom. In a fantasy setting, a witch with a good crystal orb may do the part. The plot would walk the line mutual trust and paranoia, after all, one character wants to keep her secret, the other wants to keep her life.

For the story, it would work equally well with a male or female protagonist. As far as the sexual content goes, I personally always prefer lesbian scenes to straight ones. Gender may only be story relevant, when it also involves turning a previously straight woman lesbian or at least, if she was previously in the closet.

I think a key element would also be the style of the secretly submissive woman. Outside of sex scenes, she would wear either armor or biker gear depending on the setting, while during the sex scenes, I'd rather see her in silk and lace lingerie and high heels. Mere nudity would not be sufficient for this scenario. The MC on the other hand could dress more casual. The cyberpunk version maybe wear some shirt of with a game logo.

As far as the pacing goes, I think it would be rather slow, as the setting needs tension to build up, no quick fucking. The MC would have to be both a mastermind and a daredevil to seduce this kind of woman, though this need not reflect in the difficulty of the game, meaning the character can have ideas, which the player likely wouldn't have.

As far as other characters go, I don't think there need to be many. The MC may have some trusted friends, while his love interest may have a staff of advisors, but none of either would be involved in any sexual content. The secretive nature of their romance would not allow other characters to be involved.

The reverse harem thing is unlikely to happen, which was also briefly discussed in another thread. The main reason for this is certainly that the audience for porn games is mostly male and there are few men, who want to see a woman with a male harem. Sure, there are some games with gang bangs, which I consider gay porn with a token woman anyway, but they are rather small niche. Aside from that, though, I think there is also another reason, and that, even to women it might not be as attractive as one may think at first. A woman having sex with many men is by far less prestigious than a men having sex with a lot of women. Basically, if you made such a game it would have to be in way, that doesn't make a slut out of the female protagonist. Personally I think this would be best done by giving her a lot of power - much more than any human being can have in the real world. Yet, I could think of 3 pen and paper RPG settings and one video game with women of such power:

Starcraft: Brood Love
The Queen of Blades Sarah Kerrigan needs new DNA to increase the fertility of the swarm. To combine necessity and joy, she decides to absorb worthy DNA for herself. This would likely be the most sadistic version and the scenario would suffer from turning all potential suitors into insectile horrors, but it would still be reverse harem game with female protagonist who is not some kind of slut.

Dungeons and Dragons: Dark Elves
As a matriarchal society, the drow are used to powerful women who have harems of males (and sometimes lesser females). The MC would be a high priestess, who collects the most powerful male fighters and wizards for her harem. Exalted: First Age

This would most likely be the most light hearted approach. The Amazons life in fortresses only inhabited by women. To reproduce, they sometimes have to chose worthy male characters, usually respected warriors faithful to the goddess of war, Rondra. There are no lasting relationships among amazons and no sane male character would expect one either, so there is usually no drama and jealousy involved. Drama may occur nine months later, though, if the child is male and the mother doesn't want to give him away for adaption, as her tradition demands.
