I don't really pay much attention to MC looks, although stuff like Solvalley School is kinda laying it on thick lol. Luckily a lot of games play almost exclusively from a first-person perspective, which works well in most situations. I don't mind seeing the MC face, but it's fine to me if it's avoided as much as possible. Making out scenes / french kissing (which I love and find grossly under-used in AVNs/sex games, esp. animated) obviously complicate things.

Having said that I used to have this sort of kink in Sims 4 Wicked Whims where I would pair the most hideous mofos/disgusting old creeps with a bunch of smoking hot ladies and then have all that miracle stuff happen. You'd think that's me trying to identify with characters but that's wrong since I'm young and gorgeous.

This might sound crazy, but hear me out. What if, instead of ugly bastards or empty shells, we make male MCs that are actually attractive? That way we wouldn't have to rinse our eyes after looking at the hairy ass-crack of some disgusting bastard fucking a supermodel, in a way reminiscent of "The beauty and the beast".
I'm not saying they have to be a literal Gigachad, but at least average-looking. I'm also not saying all games should be like that. I think many people find it specifically arousing when the man is very ugly, for some reason.

I feel like ugly bastard is rarer in games than hentai/manga, but I also don't play a lot of Japanese adult games because most of them suck despite me enjoying hentai/manga. People have already answered your question very well as to why ugly bastards are in porn, but why do we still enjoy the games ?

Ironically, it is only in Japanese hentai where you see a lot of diversity in male main characters. They come in various forms such as black, fat, slim, ugly, muscular, typical-looking, old, and young.

Interestingly, some of those enticing and wholesome hentai stories feature a fat old guy who actually treats the young female main character better.
