Your approach to frameworking and high concept

I guess I'm just curious to hear from devs about how they approach conceptualizing and frameworking their game design within the paradigm of: porn game.

Do you approach it by using the sex scenes as a focal point, and craft the story around that?

Or do you approach it by crafting your story, and incorporating your sex scenes within it?

It may be a subtle distinction, perhaps, but I think it's still significant, and would love to hear from those with experience here.

It depends about how I conceptualize a game. Visual novels usually gravitate towards sex scenes so the framework and mechanics behind the game go around the fact that the end goal is getting sex.

The platformers I have conceptualized, however, prioritize gameplay, with sex scenes being just an added to the game in order to improve it.

From my point of view, it depend in first hand if you want to make a porn game, or a game for adult.

It goes for porn games like it goes for porn movie/comics/books, the story don't really matter ; at least for the player. There's those sex scenes you want to put in it, and you try to link them with some sort of canvas. In such games, sex is the sole purpose of the game, everything else is just cosmetic touches.
This while, at the opposite, a game for adult is a story before everything else. Therefore, you should write your story and, once it's done, find where you can add sex without breaking the story, and while still having a result that feel natural. The story/game being the most important part, the sex scene should be the consequence of the previous action ; whatever decided by the author, or because of the player choice.
