Yes, it is possible. Depending on who you get along with, the narrative of the game changes. To unlock the threesome, you have to manage to sleep with the stepmother's friend both times she's available in the story, and also sleep with the stepmother. That depends on what you say during the dialogue moments, building either a good or bad relationship with them.
