Orgasm types tend to only count one category at a time. Try making her orgasm only through g spot for g spot orgasm (finger). Same thing for cervix, orgasm only through cervix. Use the long stick vibrator manually (auto/fix can overstimulate her for some reason, causing her too much pain). Eventually you can use penis too.
Focus on squirting through g spot for a while. Eventually you'll unlock level 2 which makes her squirt every orgasm (assuming you have occasional g spot orgasms).
the game always crashes at the same point whenever I try to load anything at the point where it says "activating scene" and I have no clue why. I set the graphics low as they go, still crashes at the exact same point every time. I've looked at Task Manager when it happens and I don't see any spike in resources that would cause a crash, and the game doesn't even create a crash report despite that setting being on. It just loads 90% of the bar, reaches "activating scene", holds for 2 seconds, then just decides to close for no apparent reason. I also didn't put in the uncensor or anything, its just the base game and still refuses to load anything.

And how do we do that?
I downloaded SB3Utility, tried opening the "sharedassets2.assets" file in this utility and it just throwed out an error in log:
"Exception has been thrown by the target of an invocation.
Array dimensions exceeded supported range."
Does anyone know why alt tabbing stops me from being able to interact with the game? I can still go back to the tab and it looks like it's fine, but my left click isn't being picked up by it (while my right click is). It's so odd
EDIT: For some reason middle mouse click is also being picked up, plus left click is being picked up but only if I'm using the vibes or licking

Go to the settings and look at the hotkeys (they exist). Unfortunately, I haven't encountered the problem you described.
What version of SB3Utility are you using?
I used SB3Utility v24.3.3.
It comes with the Koikatu! [BetterRepack RX22][Illusion] tool.
This is a different version from the latest 21.0.3 on GitHub.
