Skin Builder only change the skin tone. not the nipple shape.
So maybe you forgot to add the areola. Check in shaping if you have the areola.
Also, are you using the default nipple? Or are you using Real Nipple assets?

I am using the default nipple. In shaping I have Aereola size at -160, because otherwise they are huge and everything else at 0.

I just used the default textures that came with the assets listed in here and they look fine to me. I don't see the difference.
