2. Yes it is. Devs get around this by using publicly available images and videos AND by calling their income "tips" instead of a service. The best practice I've seen devs use on long term projects is make the game but have a completely separate "image" pack that you can customize yourself or not use at all.
3. First and foremost, women are rarely ever alone. They always have a friend or two. In this game's case, it's probably because the dev likes looking at girls.
4. Because an engine doesn't just hand you everything pre-done. Take Unity and Unreal engines for example; they are GREAT tools to use but you have to build your own systems from the ground up. It's not just scripting values, you're literally building the things that interpret those values. I'm not sure about Ren'py and how it's set up but I imagine it's similar in that it provides a toolset but doesn't actually HAND you anything. The other reason it takes so long is modders are modding a game for fun where as devs here seem to be making games for money; that means that the longer they drag it out on patreon the better their bottom line.

to 2. Well that could be true, but even if they are "publicly available images and videos" doesn't mean they are not copyrighted/licensed. And well... for calling it a tip, a tip is payed after (a good) service, if I have to pay before they even delivered something more then A Demo it's not a tip anymore I guess. From my point of view they should pay me for quality controling their games before it's even finished. But thats not how it works, I know.
to 3. Yeah, sure, but I know my Wife for many years know, and I know her friends too, and they don't turn/expose themself as sudden nympho bitches who try to influence my wife. And after playing the game, "Honey" don't seems to know their "friends" very good. But as you said "In this game's case, it's probably because the dev likes looking at girls."
