I would guess that the people aroused by NTR are the people who fear being cheated on. I personally don't fear it or care very much, if my partner cheated on me I would be sad for a little but just move on to someone else. So NTR does not feel particularly impactful to me.

But I think people hate NTR because it becomes something personal for them. What I've realized after publishing this game is that alot of people on the internet cannot differentiate fantasy from reality so it triggers their flight or fight. They try there best to stop it because they personally feel it. When in reality this is just like any other form of pornography.
I think people turn on empathy too much. It's like a battfield. It is harder for the attacking side on foreign territory because they think they are wrong than for the side that is defending. Although I may have given the wrong example.

I plan to implement NTR in my game despite not particularly caring about it - purely because it can be so evocative. The game has two major factions, a female-led aristocracy and a male-led resistance force. It is for all intents an "overthrow the matriarchy" scenario.
If the player chooses to ally himself with the sitting power, he will come across many opportunities for romancing the various female NPCs. Inevitably, the resistance force will come to take away his lover(s), and upon loss the player's lover will be seized and the scene plays out while the player is all but forced to watch. In particular, I plan to make the player's main rival the one doing the raping.
This NTR is not intended to be sexually titillating, but to hopefully make the player feel anger, contempt for the opposing force, or possibly even regret for not initially aligning himself with the winning team. Will the player vow to destroy those who violated his lover? Or possibly cut his losses and reconsider his loyalties to avoid humiliation again? It's based on the player's feelings and interpretation.
I am largely interested in the intersection of lewd games and horror games. Both are unique in their ability to evoke strong feelings. Sexual humiliation, trauma, abuse, and frustration are all touchy concepts but when harnessed properly can slap the player in the face through the computer monitor. NTR is a wonderful tool for this.
