La Vitalis: Immortal Loss, in development, and unlike the previous B-flat game allows complete anal-only with good amount of content.
There is a skill and two (cursed) items (one of them is a futa tf) that prevent vaginal penetration (though no effect on CG scenes), including 'redirecting' some enemies that always go for pussy otherwise. The MC is also a virgin.
Beside unprotected penetration, only capture CG scenes break anal-only, and they are always indicated by colored bar while grappled so easily avoidable. Capture CGs involve all 3 holes used.Trying to fill out the criteria notes myself, I belive it'd be something like that:

• Player agency and anal [✅] (All sex completely avoidable by not letting enemies grab you)
• Anal content [✅] (Mostly reduced by CGs while doing anal only, but equal if counting multiple penetration, and every enemy with penetration has anal variant)

Exposed Livestream, released on Steam uncensored with a quality translation. This is a "short butt sweet" game with a focus on public exhibition. MC is a small streamer who goes around town making spicy content in order to get rich grow her channel. Basic af gameplay loop with only a handful of CGs, but every sex scene includes a choice between vaginal or anal penetration, which I appreciated. Anal-only "virgin run" is easy to achieve but unfortunately has no impact on the story. Here's my evaluation:

• Player agency and anal [✅]
  • This game lets the player have complete control over the interactions, and by consequence allows anal-only.
• Anal content [❎]
  • It has a decent amount of anal content, as much or equal to vaginal content, and enough to be satisfying. However, the main focus is MC's exhibitionism kink.
• Dedicated anal dialogues [✅]
  • Dialogues acknowledge anal-only, with an emphasis on MC's desire to save her virginity.
• Storyline and anal [❌]
  • The game doesn't have progression for anal-only and virginity has no impact on the story.

I just finished Gran Ende Saga, which has been mentioned a couple pages back.
Quick pros and cons:

Pros:
- Full control over sex scenes, so you can go anal-only with all the female party members, wich is reflected in each one's status page

- Sex scenes change depending on if it's the first time the girl is doing something, if she has some experience and if her affection for you is really high

Cons:
- Sex scenes are entirely disconnected from the rest of the game, as they only happen when staying at inns or at your home
- Anal-only seems to block access to a lot of the sex scenes at the player home, most notably the bath scenes

Overall I'd still recommend playing it, but be aware that it's closer to a regular RPG with optional sex scenes rather than a full sex RPG.

I don't think this one can be share on this site, so, only sharing the DLsite even after release. The dev has made all their games anal-only since their first game, so, lots of anal again <3

a remake of the famous game Bible Black: La Noche de Walpurgis has been published. I haven't tried it yet, but iirc the original game had lots of anal sex.
