This isn't the first Godot game that I've seen, either! Hardcoded is a game mostly about trans girls (with all the anal content that that necessitates, though not only anal) which started out in Godot, though I'm not sure if the final version ended up being in it. Yeah look it's got the classic Jap urination/scat thing going on, there's a lot of toileting/enema content that's possible to encounter, but it's also entirely possible to complete the entire game to both endings while never seeing a single scene of that nature if you just...don't peep on the girl on the toilet and don't buy the enema syringe from the shop.

It is quite good in its anal obsession, despite the scat scenes being a bit too much. My only gripe is that there is not a lot of game in this game, just some silly grinding task completely divorced from the main theme. Plus, the dev made a lazy choice employing a clunky RPGM engine, whereby a highly interative molestation/bodypart training simulator type of engine would do a much better job at conveying the kink. For those who combine anal focus with stretching It would have been delicious to be able to observe gradual changes in the girl's hole capacity. And the player's task could simply be to utilize such a well-balanced training + psychological conditioning regime as to turn an innocent female soul into a consummate buttslut without breaking her. No need to pretend you're actually an alchemist refining some BS potions. Just you and that cute ass of yours, young lady, how about that? But then again, the mosaic censorship would ruin that anyway.
