I figured out the combo... Immortality + Death + Ghost + Death will turn her into a ghost, then give a cup of Sex and you'll get the anal scene.

Only Transformation so far are Devil, Angel, Ghost, Cancer, and the Devil + Angel. At least so far that I found.

Extracting the .PCK shows there should be a total of 6 transformations / adult scenes in the game:

• angeldemon_sex.tscn
• cancer_sex.tscn
• default_sex.tscn
• ghost_sex.tscn
• heaven_sex.tscn
• hell_sex.tscn

Combinations to achieve these should be giving the girl the following drinks:

• "Angel" + "Demon" + "Sex"
• "Cancer" + "Sex"
• "Sex"
• "Ghost" + "Death" + "Sex"
• "Heaven" + "Sex"
• "Hell" + "Sex"

Note that these combinations may fail to produce a result if other drinks have previously been consumed. Easiest method to fix that is just exiting to the main menu and starting fresh. There are other drinks that produce a unique response, but these appear to be text only.
