Nobody knows why, but here's the changelog:

V3 > V4 fixes:
- Quicksaving function (F5) / fully reworked save system (should be much more stable)
- Doors are no longer closes up behind the player, all interactive objects are saved - you can return back and find any objects you've missed
- Added Difficulty setting (the hardest one unlocks after beating the game for the first time)
- Fixed bug on sex scenes, if player was "captured" while Crouched/Crawling
- Fixed bug with mouth-texture visible, after "forcing out" of 1-st person mode
- Fixed bug with puzzles not combining into 1 book
- "Pee girl" desk puzzles can be reset with "R" button, also fixed problem where this puzzle is "solved", but not "triggering" as solved

Yes it still ends at the same place as that is Episode 1

v3 has mostly QoL bugs but no game-breaking ones. Don't know about v4 but with v3 you should be safe.

I have tried every method metioned to load any save, I get only fatal errors.
So if I die, or just want to quit, I have to start all over again.

v4, NewGame+ black screen after first cutscene. Can I 100% everything without NG+ ??

edit : somehow NG+is working on my 2nd playthrough with 8/7 secret found *facepalm. Awesome Game Awesome Bug, but i have to delete this game since there is no reason to repeat it again.

I've had better success with quick save working on v4. They both are really buggy. I had a moment is v3 where suddenly the zombie and the heroine were jammed up in to the ceiling floating and not able to escape.
