Not sure how to explain. im on day 345 and the story doesn't progress any more. Every day is basically the same the same and i have tried everything (i think).

Well the most important info that is needed is who is your GF and what does "Cunning Plan" says.
Any other open quest would also be helpful.

My girlfriend is Olivia. "Cunning plan" is not finished. Other open quests are "Arachnophobia", "Blog" and "New job".
Then a few quests says they are finised for this version of the game.

Olivia content ended in v.10 and all new content in v.11, v.12 and v.13 is only for the Lisa GF routes.
This is why nothing else is happening in your game you are at the end of content.

If you want all the new content then yes and go 1 of 2 Lisa GF routes.
If you need to know how to get one of hers then its at the bottom of this page.

You have to progress other quests to unlock the triggers for more during the spider quest.

I'm pretty far in the mom story i've reached the final 'movie' with the sister and talked to aunt Kira who sleeps on the couch now but still can't enter moms room between 00:00 and 03:00 no matter what i do. Kind of lost. Anyone any idea? Thanks.

As for sleeping or trying to sleep in her room before you do she starts the 5th movie you chose to talk about the movie instead of talking about sleeping in her room.
You will get the chance again after the 5th movie is done and mom starts her new job.

I have two issues I hope people can help with.

1. I'm stuck on getting Ann to give Max a handjob. I've done it in previous playthroughs, but for some reason the conversation with Kira about Carpel Tunnel has never appeared. I've been stuck on naked massage for weeks.

2. Spanking Alice. I've been spanking her in private for weeks also and never seen the option for sex instead.

Any suggestions on what's gone wrong would be much appreciated.

I explain, after the aunt's photo shoot, she says "we will be able to put this back when no one is hugging at home" but I do not manage to have a relationship with her or half at night in the living room. ditto for alice and massage in the living room. thank you for your future help and sorry if I'm not on the right post

You must progress in other quest to get the triggers like cunning plan and new job.
Keep showing up at her school to.

What does the new job quest says on it?
Make sure all mood is as high as it will go.

Its Alice not alisa.
