I can't recall a game that has a focus on clown/jester girls, but there's a few games that has a jester as a side character

Tower of Trample - One of the girls is clown/jester themed (Jessica), just keep in mind she is the chubbiest of all of them (I don't mind it, but some people do)

Domina - Similar to Tower of Trample, one of the girls is clown/jester themed, she, however, also has a focus on gore and torture, so keep clear if you are not into that
